This is the amplifier's switching frequency. There can be some visible on the output, but I'm not sure about 8 Volts. That sounds a little high to me, you could have some damaged components in the output filter.

When using the app, there are two places to adjust levels. There is the analog gain on the unit and there is digital gain that can be adjusted within the app.
And just to make everything sync'd up, are you turning App Mode on/off on the unit?

The PV6BT requires +15V DC.

Yes, take the signal from the Record Out jacks. They are pre-master volume...so the master volume doesn't affect the level. No...they are only cut from the amplifier outputs. If you look at the block diagram in the manual, you will see the DSP is after the analog mixer.

That is a two tip 1/4" jack for higher current applications. The other pin is the switching pin...but it should have no connection to the ground pin.

Going off of memory...no.
Some of them had bridged outputs, some had Speak-on outputs and some only had 1/4" outputs. I believe the output jack configuration was the main difference.
